Position Details Full‑time, 8:00 am – 4:30 pm, Emergency Department, LCSW required. Responsibilities Conduct screening and assessment activities to meet the needs of the discharged patient. Apply assessment findings to prepare a patient‑centered plan of care. Apply and comply with all regulations and utilize resources available to patients to manage their continuum of care. Qualifications & Requirements Master’s Degree in Social Work (MSW) – required. LCSW license required. Experience: No experience required. Supervisor Experience: No experience required. Preferred Experience: 2–5 years. Licenses & Certifications Licensed Clinical Social Worker (LCSW).
