Job Description

Maplewood Brewster LLC, a senior living community in Brewster, MA, is seeking a compassionate and dedicated RN/LPN Wellness Nurse to support residents in our assisted living and memory care neighborhoods. In this role, you will provide resident-centered nursing care, partnering closely with residents, families, physicians, and the care team to promote wellness, safety, and quality of life. The RN/LPN Wellness Nurse is responsible for ongoing clinical assessments, monitoring changes in condition, and implementing individualized care plans tailored to each resident’s needs. You will administer medications and treatments according to physician orders, accurately document all care in the electronic medical record, and ensure compliance with state and federal regulations. You will collaborate with caregivers and other team members to support daily care, participate in care plan meetings, and help guide staff in best practices for assisted living and memory care. This position is hands-on and on-site, involving daily interaction with residents living with varying levels of physical and cognitive abilities, including dementia and Alzheimer’s disease. You will play a key role in fall prevention, infection control, and early identification of health changes, coordinating with providers and families as needed. Education of residents, families, and staff on wellness, chronic disease management, and safe care practices is an important part of the role.
