Vetted is seeking a RN - Case Management for a travel job in Bremerton, Washington .

Must have 2+ years of experience.

This contract pays approximately $9,608/month gross.

Assignment details:

• Contract length: 8 weeks
• Shift length: 8 hour shifts
• Hours per week: 40
• Start Date: ASAP

Job responsibilities:

• Document care delivered, patient response, and communications in the medical record.
• Work 40 hours per week with 8-hour shifts.
• Educate patients on treatment plans, follow-up instructions, and self-management strategies.
• Perform medication reconciliation activities as directed and communicate discrepancies.
• Support care coordination activities across providers to promote continuity of care.
