Maplewood Brewster LLC is seeking a compassionate RN/LPN Wellness Nurse to join our Healthcare & Medical Services team in our assisted living and memory care community. In this role, you will play a key part in supporting resident-centered care, focusing on wellness, clinical quality, and meaningful connections with the seniors we serve. As an RN/LPN Wellness Nurse, you will conduct ongoing assessments of residents’ physical and cognitive status, monitor changes in condition, and collaborate with physicians, families, and the interdisciplinary team to ensure individualized care plans are developed, implemented, and updated. You will administer medications and treatments in accordance with state regulations and company policies, maintain accurate and timely clinical documentation in the electronic medical record, and uphold infection control and safety standards throughout the community. You will provide daily wellness support by monitoring vital signs, responding promptly to resident health concerns, and coordinating outside services such as home health, rehabilitation, and hospice when needed. In partnership with our care team, you will help guide and mentor caregivers, ensuring they understand residents’ care plans and are following best practices in senior and memory care. You will also participate in care conferences, quality improvement initiatives, and regulatory compliance activities to support high standards of clinical excellence. This position is highly interactive and resident-facing. You will build trusting relationships with residents and their families, offering education on health conditions, medications, and wellness strategies, as well as providing emotional support as needs change.
